Police Check Procedure

Every year, all CMHA volunteers must obtain a police check. The page contains the procedure for obtaining the check. This applies to the following positions:

  • coaches
  • assistant coaches
  • trainers
  • team managers

Note - Police Checks obtained in the spring for coaching Soccer/Football are not valid.

Note - Everyone must obtain a police check, regardless of your occupation.

 

Steps

 

Please follow these steps:

  • Download and print the form "POLICE RECORDS CHECK FOR SERVICE WITH THE VULNERABLE SECTOR" by clicking HERE.
  • Fill out the form:
    • agency = Cumberland Minor Hockey Association
  • Obtain a CMHA association letter from your convenor, otherwise, you'll have to pay a $15 processing fee.
  • You must appear in person with the documentation and originals of the required identification to the Orleans branch or downtown branch of the Ottawa Police Services.
