How To Obtain a Refund or Rebate of Registration Fees

** Refunds are not automatic **

** Refunds are only issued upon written request directly from the parent, not the coach or convenor. **

 

All requests for refunds and/or rebate must be made in writing to the C.M.H.A. District XII Registrar. Rebates apply in situations where there is more than one child registered.

All refunds are subject to the following refund schedule:

  • Request prior to October 1: 100% less $25.00
  • October 1 to October 31: 80% less $25.00
  • November 1 to November 15: 65% less $25.00
  • November 16 to November 30: 50% less $25.00
  • December 1 to December 31: 25% less $25.00
  • After January 1: No refund applicable.

 

All requests must have the following information:

  • date of request
  • player's name
  • date of birth
  • division of player (IP, Novice, Atom etc..)
  • amount paid
  • mailing address
  • telephone number

A parent or a legal guardian must also sign the request.

The refunds will be subject to the "Refund Schedule" shown above, and a $25.00 "Administration Charge".
