All requests for refunds must be made using the following form:

Telling your coach or Convenor that you would like a refund is not sufficient.

The refund value will be determined by: the “Refund Schedule” below; be subject to a “Administration Charge”; and conditional on the return of all CMHA issued equipment: 

REFUND SCHEDULE:

Request prior to September 30	100% less $25.00
October 1 to October 31	75% less $25.00
November 1 to November 30	60% less $25.00
December 1 to December 31	40% less $25.00
After January 1	No Refund
